- Plan long-term care

- Provide other services, as requested
Home Health Care Services
Level One: Companion/HomemakerCaregiver ensures the client is safe yet provides no direct care. Duties may include:
- Engaging client in friendly and respectful conversation and/or activities.
- Performing light housekeeping tasks, including cleaning (pick up spills, dishes, vacuuming and sweeping small areas), changing linens, and doing laundry.
- Helping locate and bring to client items in room when needed (including when getting in and/or out of bed).
- Assisting client with using telephone, opening mail or organization.
- Preparing and serving meals.
- Encouraging client to eat and/or drink more fluids.
- Monitoring client while he/she feeds themselves.
- Reminding client to use cane, walker, crutches or wheelchair, as appropriate.
- Reminding client to bathe/shower.
- Assisting with grooming and personal hygiene, including but not limited to shaving, oral hygiene, skin care, and nail care (for non-diabetics).
- Assisting with dressing or undressing.
- Reminding client to take medications (caregiver may not touch medications).
- Running errands for client (without client accompanying, subject to mileage if using caregiver's vehicle).
- Daily maintenance of plants and/or animals, if applicable.
- Redirecting client, as needed.
- Making notes of visit in caregiver log.
- If client is in a facility, contacting staff when client needs care and/or attention.
- Providing other tasks, as requested and approved by supervisor.
Level Two: Home Health Aid and/or Personal Care Attendant
Caregiver assists with life activities and direct personal care, pursuant to doctor's order, when applicable(must be reviewed by the doctor at least annually), written instructions and/or an established plan of care. In addition to duties described in Level One, caregiver duties may include:
- Assisting with bathing/showering (in bathroom or bed).
- Assisting client with toileting and/or changing briefs.
- Transferring client (caregiver will only attempt to transfer client alone if client can assist by supporting some weight).
- Assisting client with ambulation, using gait belt, cane, walker, crutches or wheelchair, as appropriate.
- Accompanying client to appointments or on errands (subject to mileage if using caregiver's vehicle).
- Assisting client with eating meals and drinking fluids to avoid aspiration.
- Assisting client with exercises, as directed.
- Taking and recording client's vital signs.
- Notifying supervisor of change in client's condition or need for higher level of care.
- Providing other tasks, as requested and approved by supervisor.
